Enrolment Forms

It is essential that your child is enrolled into O.S.C.A.R.S. and booked into your required session(s) prior to attending. Even in emergency situations, we cannot have a child in our care unless we have all the required paperwork.

In order for your child to attend O.S.C.A.R.S., we must have a completed O.S.C.A.R.S. Enrolment Form. Enrolment Packs (containing the Enrolment Form) can be obtained from O.S.C.A.R.S. or the Unley Primary School front office. We can also send you an O.S.C.A.R.S. Enrolment Pack in the mail (free of charge). Alternatively, the documents contained in the Enrolment Pack are now available for download below.

Please Note: We have a $50 Once-off Family Enrolment Fee, which covers all administration, food costs, purchasing supplies for the children, etc. As O.S.C.A.R.S. is a not-for-profit organisation, we rely on payment of fees and the Once-off Enrolment Fee to cover all our costs. The enrolment fee is only ever paid ONCE PER FAMILY (no matter how many children you have using our service over time).

Once you have completed and returned an Enrolment Form, we will then book your child into Before and/or After School Care as per your booking schedule on Page 2 of the form. If you wish to use our Vacation Care service, we must ALSO receive a Vacation Care booking form (see 'Vacation Care' ).

All enrolments will be accepted according to the Commonwealth Government 'Priority of Access Guidelines'. If a place is not immediately available at the service, the child may be put on a waiting list. Details can be found in our Priority of Access policy.
